I'm not aware of any current exchange based in the USA. But, that doesn't really matter. I have purchases on a variety of international exchanges over the years and simply sent my coin to cold storage.
LocalBitcoin, coinbase, and face-to-face through Craigslist are the most common methods of buying/selling in my area of the US.
There are a few ATMs around the country, but none where I am.

I've used Coinbase to convert my USD to BTC, and BTC-e to trade cryptocurrencies. No problems yet, and the Coinbase platform is evolving quickly.
Coinbase does charge a premium, but it's pretty small, for the convenience it offers. Once Circle opens up to the public, we'll see if they can beat Coinbase at their own game.
